The use of social determinants of health in the field of Intellectual Disabilities is a relatively new endeavor.  Integrating variables related to clinical and non-clinical needs enhances an individual’s ability to participate in their own care and become more self-directed.  Using data analytics to identify patient risk, identifying community-based resources, tracking of health outcomes, and making care intervention a collaborative process are examples of the ways clinicians and providers are using SDOH to make healthcare a more collaborative process. This presentation will address how to implement each of these variables and give real life examples of how this can be done effectively.
